The Home Office Identity & Passport Service, HM Revenue & Customs the Driver and Vehicle Licensing Authority, Department for Work and Pensions, and the Job Centre, will not accept a photocopy of your Deed Poll.
In addition to sending your Deed Poll to the organizations with whom you need to change your name, you will also be required to send a separate supporting letter. If you are changing your child's name and you share parental responsibility with another person, you will be required to submit a specifically worded letter (with the Deed Poll) bearing the signature of all persons who retain parental responsibility for the child.
