The Home Office Identity & Passport Service, HM Revenue & Customs the Driver and Vehicle Licensing Authority, Department for Work and Pensions, and the Job Centre, will not accept a photocopy of your Deed Poll.
In addition to sending your Deed Poll to the organizations with whom you need to change your name, you will also be required to send a separate supporting letter. If you are changing your child's name and you share parental responsibility with another person, you will be required to submit a specifically worded letter (with the Deed Poll) bearing the signature of all persons who retain parental responsibility for the child. The supporting letter is also required if you are changing your child's name without the consent of an absent parent. The free letter templates are available for download post application submission. FREE Digital Archiving Service Every Deed Poll is automatically stored in a digital vault (powered by Microsoft Business Sky Drive®) for a period of 20yrs. Misplacing a Deed Poll is a very common occurrence, thus this service enables clients to obtain a copy of the archived Deed Poll at any time in the future.
