Obtaining a copy of your lost deed poll document
There is no register of name changes in the United Kingdom so if you have lost your deed poll or it has damaged beyond use you cannot get a copy of the original document like you can with birth, death, and marriage certificates.
If this is the situation you find yourself in we can prepare a special replacement document for you to use. The replacement deed polls we provide have exactly the same legal validity as your original deed poll and can be used for any purpose including—
- applying for a passport or driving licence in your new name
- registering for marriage
- obtaining DBS check.

Why can't I get a copy of my original deed poll?
A deed poll is a legal document, not a certificate, so there is no register or database of deed polls from which a copy could be obtained. If you lose your deed poll the only option available to you is to sign a new specially prepared replacement document.
Will the replacement deed have the date of my original name change?
Yes. The replacement documents we prepare include a section on the document which lists when you originally changed your name.
Can I get a replacement deed poll for a deceased relative?
Unfortunately not. As a deed poll is a legal document it must be executed (signed in the presence of a witness) by the person who changed their name in order to have legal effect. If the person who changed their name is now deceased it is obviously not possible for them to execute the replacement document and it would therefore not be valid.
