Letter of consent

You only need to download the appropriate letter if you share parental responsibility (PR) with another person. We don't need to see a letter of consent if you have Sole PR.

Note: You can use the template below to write your letter of consent by hand, if you don't have a printer.

You can send your letter of consent to us by:

Sample Letters

Joint PR (everyone with PR consents)

This letter is required if everyone with PR consents to the change of name.

Joint PR (absent parent)

This letter is required if the mother or father is absent, or if an exception applies.

Click here if the absent fathers whereabouts is not known.

Click here if the absent fathers whereabout is known but you do not wish to contact him because an exception applies (e.g. history of domestic violence or child abuse, etc.)

Click here if the absent mothers whereabouts is not known.

Named father is not the biological father

This letter is required if the person recorded on a child's birth certificate as the father is not the biological father.
