6. Changing the name on your child's passport

If you wish to change the name on your child's passport (or you are applying for a first passport in a name that is different from the name on your child's birth certificate), you will need the consent of everyone with PR (parental responsibility).

We will also need to see evidence of consent from those with PR before we can issue your child's Deed Poll. 

Therefore when you apply to us for your child's Deed Poll, simply mail us a letter of consent signed by everyone with PR. This letter will be returned to you so you can send it to the passport office along with your child's Deed Poll when changing the name on your child’s passport.
