4. Applying for a Replacement Deed Poll
If you want to apply for a Replacement Deed Poll you will need to complete our ‘Replacement Deed Poll’ application form – but only if you have not changed your name since changing your name using the Deed Poll or Statutory Declaration that you have lost.
If you’re applying for a Deed Poll because you’ve lost a Statutory Declaration, don’t be put off by the fact the application form (Replacement Deed Poll) suggests it can only be used to replace a Deed Poll.
Because you’ve already legally changed your name (in this case by Statutory Declaration) and have been using this new name since that date, you can use our Replacement Deed Poll application form to apply for a Deed Poll to replace your lost Statutory Declaration.
This Deed Poll will be accepted as official documentary evidence of your change of name by all government departments, companies and organisations.
However if you’ve changed your name again since the original name change (the name change that features on the lost Deed Poll or Statutory Declaration) then you should call our Helpline (see top of page for the telephone number).
