3. Obtaining a Replacement or copy of your Statutory Declaration

When we hold someone’s Deed Poll in our archives we can only issue a legal copy of that Deed Poll to the person named on the original document.

We can, however, issue a legal copy of a child’s Deed Poll held in our archives to anyone with PR (parental responsibility) for the child or to the child – but only if that child is still under 16 years of age.

If you aren’t the parent that applied for the child’s original Deed Poll we will need to see documentary evidence that you have PR (e.g. your child's birth certificate, marriage certificate or a court order). If you’re unsure what documentary evidence you will need to provide, please call our Helpline (see top of page for the number).

If you are researching your family history and are looking for evidence of a family member's past name change, please click here.
